1. Fill out this online form to RSVP
2. You will be *automatically sent the Ski Trip Packet when you RSVP:
    a. Keep the info sheet for your reference
    b. Fill out the medical waiver and return to FPC, attn: Erika Hagan, Administrative Assistant.
3. Bring a check made out to FPCNC in the amount of $175.00, with "Youth Ski Trip" in the Memo.
